In clinical studies:

NO DOSAGE ADJUSTMENT FOR MOST PATIENTS

– The elderly
– Patients with hepatic insufficiency
– Patients with mild to moderate renal insufficiency*

INVANZ is eliminated primarily by the kidneys

– No drug interactions reported in clinical trials**

Patients with renal insufficiency: INVANZ may be used for the treatment of infections in patients with renal insufficiency. In patients whose creatinine clearance is
>30 mL/min/1.73 m2, no dosage adjustment is necessary. Adult patients with advanced renal insufficiency (creatinine clearance ≤30 mL/min/1.73 m2), including those on hemodialysis, should receive 500 mg daily.

Because of the small effect on half-life, the coadmininstration with probenecid to extend the half-life of ertapenem is not recommended. Decreased serum levels of valproic acid with coadministration of INVANZ have been reported as post-marketing experiences. Careful monitoring of serum levels of valproic acid should be considered if both drugs are to be coadministered
